she has obviously undergone "labiaplasty" - a medical procedure whereby the labia (lips/flaps etc) are surgically removed or reduced in size.
compare the labia size from older set like this:

You need to look closer. In the earlier pictures, Ekaterina was more prone to frolicking before her photos. I do miss seeing her body so aroused.
